We are a locally privately owned practice that provides specialty medical exams to the community. These exams have a highly technical component which we are experienced in providing.
Some examples are FAA Flight Physicals, USCIS Exams for Immigration and other specialty USCIS Exams; DOT exams , and US Coast Guard and Merchant Mariner physicals.

Dr. Weitzman has been in practice for over 25 years. The clinic originated from requests from our patients in the Myrtle Beach, North Myrtle Beach, Conway and Grand Strand communities for these exams. Collaborative & Innovated Care was created to accommodate the need for our community.
About the Doctor
Dr. Weitzman is certified by a number of federal and state agencies to complete these physicals. He and his family are long time Grand Strand residents, working and going to school in the community.
Our Services
This is a USCIS physical or I-693 exam is a required step for immigrants to change their status or become US citizens.
This is a physical required for workers of the state transportation that is done upon the request of the employer.
This multi page form must be filled out in the entirety. The cost of these are $250 (?). If you need a drug screen, you will be sent to the lab.
This is a physical required by the Federal Aviation Administration exam that covers allowing a person to Fly different types of aircraft.
